A restaurant reservation form gives guests a consistent way to request a table and provide the details a front-of-house team needs to check capacity and prepare for the visit.
A request form is not the same as a live booking system. Unless availability is checked in real time and a confirmation is issued, the form should clearly say that the preferred date and time still require restaurant approval.

State prominently that submitting the form does not reserve a table. Explain who reviews requests, when guests should expect a response, and what counts as confirmation—such as a phone call, email, or text from the restaurant.
Do not expose service times that the restaurant cannot consistently review. Same-day requests, large parties, private dining, and holiday services may need a direct-contact or separate inquiry process.
Replace the sample time windows with the restaurant's actual lunch, brunch, dinner, tasting, or late-night services. Define the largest party this general form can accept and direct larger groups to a reviewed events or private-dining workflow.
List only seating options the venue can offer. Preferences such as patio or window seating should not be presented as guaranteed, and outdoor requests should explain weather-related changes where relevant.
Invite guests to identify allergies or dietary restrictions in plain language, but do not promise an allergen-free environment unless the restaurant has verified procedures supporting that claim. A form submission must not replace direct staff communication about a severe allergy.
Train the responsible team to review notices before service and confirm what can or cannot be accommodated. Avoid collecting diagnoses or unrelated medical details, limit access to staff who need the information, and follow the restaurant's retention practice.
Provide a respectful way to request step-free access, wheelchair space, quieter seating, or another practical accommodation without requiring a diagnosis. Publish an accessible phone or email alternative and verify the physical details staff communicate.
Ask about high chairs or child seating only when it affects table preparation. Never assume that a seating preference, accommodation, or child seat is available until the restaurant confirms it.
