What this Vendor Onboarding Form is for
A vendor onboarding form gives an approved or conditionally approved supplier and the internal sponsor one structured place to prepare the operational handoff. It helps procurement, finance, accounts payable, operations, facilities, IT, security, privacy, legal, and business owners see the same high-level relationship, readiness, and review context before setup begins.
This form starts after selection and before activation. It is not a vendor application, due-diligence report, contract, statement of work, purchase order, credit decision, tax opinion, payment instruction, security authorization, compliance certification, or vendor-master record. Keep those decisions and records in their governed source systems.
