Lead Capture form step

Use the Lead Capture form step when you want to turn responses into a lead before the respondent reaches the completion card.

Unlike standard answer blocks, Lead Capture is a dedicated form step built for conversion moments. It can sit in the normal sequence, act as a Jump Logic destination, and optionally allow respondents to skip it.

How it works

  1. Open the Questions step and add Lead Capture from the block palette.
  2. Place it after the questions that qualify or segment the respondent.
  3. Choose which fields to show, which of those fields are required, and whether consent is required.
  4. Set the gate behavior to decide whether respondents must submit the form, may skip it, or can flow past it normally.
  5. Customize the title, description, and button labels, then continue to Completion.

Available fields

  • Name and Email are enabled and required by default.
  • Optional fields include Phone, Company, Country, State / Province, and City.
  • Each enabled field can be marked required independently so the step only asks for the information needed to capture, qualify, or route the respondent.

Gate behavior

  • Hard gate: Respondents must submit the form before continuing. This is the default behavior.
  • Soft gate: Respondents see the form but can use the skip button to continue without submitting.
  • None: Use the step without forcing a lead decision at that point in the flow.

Copy and consent

  • Title defaults to Before you get your results...
  • Description defaults to Tell us where to send your personalized results.
  • Submit label defaults to Get my results
  • Skip label defaults to Skip for now
  • Consent label defaults to I agree to receive updates.
  • Turn on Consent required when the respondent must explicitly opt in before submitting.

Notifications

  • Turn on lead notifications if you want QuizFlow Labs to email you when a new lead is captured.
  • You can set the notification email, identify the client name, and choose whether the email includes answers.
  • Answer detail can be sent as a summary or full payload.

Logic & routing

  • Lead Capture can be the next step in the normal flow or a destination from Jump Logic.
  • Routing into lead capture is forward-only, just like other question destinations.
  • After the form, respondents continue to the configured next step, usually Completion, where you can drive the right next action with tailored copy and CTAs.

Tips

  • Put Lead Capture after the highest-signal questions so the form appears once you have enough context to qualify the respondent.
  • Use Soft gate when you want a lower-friction conversion path and Hard gate when contact details are required for the next step.
  • Keep the enabled fields minimal. Every extra field adds friction.
  • If your completion message references lead data, confirm the form appears before Completion and that the needed fields are enabled.
